Who may qualify
- Chronic Care Management generally requires two or more chronic conditions expected to last at least 12 months (or until death) that place you at significant risk.
- Principal Care Management is for one serious, complex chronic condition that meets applicable Medicare criteria.
- Monthly medical visits are scheduled when medically necessary — not every patient receives a monthly physician visit, and visits are not automatically covered because you have a chronic condition.
- You don't need to be a RapidMDnow primary care patient to join CompleteCare — you can keep your current doctor.
- Enrollment requires a clinical review and your consent. Copays, coinsurance, or deductibles may apply; our team will explain any costs before you enroll.
Eligibility depends on qualifying medical conditions, clinical needs, and insurance coverage. RapidMDnow does not guarantee coverage, reimbursement, eligibility, or clinical outcomes. Final eligibility is determined by our clinical and billing teams.
